For New Members - Forms A, B and D

For Past Members who were in Flight C, D and Peewees last season - Forms A,B and if you want to be evaluated form D.

For Past Members who were in Flight A or B last season - Forms A,C and if you want to be evaluated form D.

After you have completed each form, please mail them to the address specified in the forms with payment included.
